Return Instructions:

Cut out the return shipping label located at the bottom of the printed Return Form.

Place the Return Form and the returned items in a secure package. You may use the original package if available.

Seal the package with clear packing tape.

Securely affix the return shipping label to the package. Make sure all previous labels are covered or removed.

Mail your package in one of the following ways:

  • Your mailbox
  • Any public U.S. Mail collection box
  • Any U.S. Post Office
